It revolves being culturally competent and by that understanding religion, mental health, and the combination of both by identifying religious competence as part of cultural competence. This involves implementing as well as determining the “patients’ psychological and existential frameworks; it can also play a key role in determining behavioral variables (which, in turn, influence physiological variables) that have a direct bearing on mental health” (Whitley, 2012). My strengths would be to initially identify the patient’s spiritual preferences, being attentive by being an active listener to the patient’s spiritual needs. Moreover, spirituality is essentially an unchecked vital sign that is essentially to all patients regardless of their religion. However, weakness mainly finding consultation for patients whom are very particular about whom they speak. Also being exposed to different patient populations within med-surg that have different capacities which can be difficult to honor their wishes but do so to the best of my ability. When it comes to patients, it’s important to improve patient care through “taking a spiritual history, respecting and supporting spiritual beliefs, challenging beliefs, praying with patients, and appropriate consultation with clergy” (Koenig, 2008). This is something I will continue to work on as time goes on.
If I were a patient with the capacity, I would coordinate with the health care team about my preferences and needs and be updated on all of the happenings regarding my plan of care. However, should I lack the mental capacity to make such ethical decisions, my advocate would be that of my parents fulfill my wishes when it comes to my plan of care. They will be informed about all options when it comes to treatments and seek both a realistic, viable alternatives should they be present.

Adverse events such as medication errors, falls, injury and faulty nursing interventions can be an issue in hospitals, especially for staff nurses with large nurse-to-patient ratios and a low number of employees. While most hospitals have safety standards, some nurses may neglect safety measures or fail to implement them due to not feeling a lack of urgency or importance for safety. Safety culture in nursing may help combat this as it involves the internalization of promoting safety in the ward and can reinforce the need for nurses to provide and implement safety measures.
In order to formulate my question, I utilized the PICOT formula for prevention which is: P-I-O-C or population, intervention, outcome and comparison or control. I used the one for prevention over the standard PICOT as I am focusing on an intervention of adverse event reduction compared to the remedy or treatment of an illness or problem. I selected staff nurses in primary care settings such as hospitals and medical centers as my population as they are at increased risk for adverse events and are larger in population compared to nurses in clinics and community health settings. I selected safety culture as my intervention as studies have increased on its implementation, but could still use more research to contribute to further findings.

Re:Topic 3 DQ 1A vulnerable population is a group that is more likely going to develop health-related problems. They are more likely to have problems accessing health care, and are more likely to have shorter life spans and poorer outcomes. Not all persons that are at risk for poor health are vulnerable. To be vulnerable, there must be aggravating factors that place them at greater risk for on-going poor health.
At risk populations are sometimes associated with people that exibit concerns with communication, medical, independence, supervision, and transportation services. The key is based on understanding how a person functions on a daily basis (Minnesota Department of Health, 2016). At risk populations would be those with cancer, people with transplants, people with HIV, pregnancy, and diabetes. Some groups that would be considered vulnerable would be the econmical disadvantaged, racial and ethnic minorities, the uninsured -low income children, the elderly, the mentally ill, and the homeless.
In L&D, we see a group that is vulnerable, the pregnant homeless woman. There is a pattern that typically follows; no support system, substance abuse issues, and mental health issues. This group has a hard time advocating for themselves due to the systems many hurdles and also the mear fact, these women tend to have mental health unstabilites. Substance abuse further complicates their plight. We have services in place to help them find a living situation that would be in their best interest, however, they are still able to accept or decline help. We see this population coming in to the OB triage frequently to obtain some prenatal care, which is better than none. It is frightening for them, they know their child will be taken away from them, they also may fear repurcutions of their drug use concerning their baby. In South Carolina, drug abuse by a pregnant woman is the toughest in the nation. In 1997, a South Carolina court ruled that expectant mothers who use drugs can be convicted of child abuse, dozens of women in that state have faced charges due to their drug abuse (“Pregnant and homeless,” 2001). Being part of this vulnerable population puts the woman and unborn baby at-risk. These women fall through the cracks without proper advocacy. There is so much that needs to be done to help this woman find success in her life, it is overwhelming for the nurse and professionals trained in this area, we can only imagine how heavy this burden must be for the woman when she stands alone.
Amy Winehouse had borderline personality disorder, this was her vulnerability, this made her susceptible to self-destructive behaviors. Freddy Mercury (Queen) was a member of an at-risk population, he was a gay man with HIV. This made his body vulnerable to AIDS.
