Historical Development of Nursing Research and Knowledge

           Nursing today is more than just caring for the patient, it has now become imperative that nurses become involved in research in order to find the best practice in caring for those patients.  We can no longer depend on the fact that “well this is what we have always done”. We now need to do more. In the past thirty years nursing research has grown remarkably providing nurses with an evidence-based arena in which to practice.  It was not always that way and perhaps nursing research started back in the days of Florence Nightingale. In the day when nursing care in hospitals was disorganized, unsanitary, because they did not know any better, and lacked a scientific foundation, Florence Nightingale came along with her education and expertise. Her first major achievement was the fact that she recognized the importance of collecting scientific data, and her second major achievement was implementing nursing education. Because of her skillful analyses, she was instrumental in promoting changes in nursing care and in public health. Nursing care and nursing research has evolved since then over the years to become what it is today.  By the 1970’s there were nursing journals established including Advances in Nursing Science, Research in Nursing and Health, and the Western Journal of Nursing Research (Polit & Beck, 2012). By the 1980’s and beyond technological advancements, like computers, opened up a whole new arena for research. Nursing and clinical research continues to grow today demanding promotion of excellence in nursing and improved outcomes for our patients.

           Being a baccalaureate prepared nurse in the health care field today means that one has the expertise to study, practice, and educate others in the importance of evidence-based care for our patients. Nursing in the workforce with a bachelor’s degree has increased skills in understanding and using research applications, clinical reasoning and judgments, case management, leadership, and health promotion. Nurse executives today along with leading nursing organizations, and the Magnet organization, among others, have recognized the need to increase the educational level of nursing to at least a bachelor’s degree (Hendricks et al. 2012). Nurses today all over the U.S. and abroad are encouraged to attain at least a bachelor’s degree in order to keep up with the growing need of having a strong basis for critical thinking, nursing research, and evidence-based practice. Evidence-based knowledge is a combination of theoretical, silent practical wisdom, intuition, experience and personal development (Back-Pettersson, Jensen, Kyle’n, Sernert, & Hermansson, 2013). Having a baccalaureate degree and education in research, through a formal educational program is associated with better attitudes and better knowledge of research and helps nurses to expand their professional networks in relation to coworkers, leadership, and other participants in the health care industry.

Case Study:

Until recently, the risk of criminal prosecution for nursing practice was nonexistent unless nursing action rose to the level of criminal intent, such as the case of euthanasia leading to murder charges. However, in April 1997, three nurses at the Centura St. Anthony Hospital, outside of Denver, were indicted by a Colorado grand jury for criminally negligent homicide in the death of a newborn. Public records show that one nurse was assigned to care for the baby. A second nurse offered to assist her colleague in caring for the baby. A third nurse was a nurse practitioner in the hospital nursery. Because the baby was at risk for congenital syphilis, the physician ordered that the nurses give 150,000 units of intramuscular penicillin, which would have required five separate injections.

In relation to other problems the same day, the baby was subjected to a lumbar puncture, which required six painful attempts. To avoid inflicting further pain, nurse two asked the nurse practitioner whether there was another route available for the administration of the penicillin. Nurse two and the nurse practitioner searched recognized pharmacology references and determined that intravenous administration would be acceptable. The nurse practitioner had the authority to change the route and directed nurse two to administer the medication intravenously rather than intramuscularly. Unrecognized by the nurses, the pharmacy erroneously delivered the medication, prepared and ready to administer, in a dose ten times greater than was ordered—1.5 million units. As nurse two was administering the medication intravenously, the baby died. The Colorado Board of Nursing initiated disciplinary proceedings against nurse two and the nurse practitioner, but not against nurse one. The grand jury indicted all three nurses on charges of criminally negligent homicide but did not indict the pharmacist (Calfee & Plum, 1997).

Another important element is the state of the health care services that are provided; they need to be of high quality despite the wide coverage. This is done by excluding the pre-existing conditions of discrimination and prohibition of non-important elements. Through this, some of the provisions offered are not only enhanced, but also comprehensiveness is achieved. State flexibility is the other element, and hence some of the programs need to be established so as to ensure that the low income individuals are in a position to access the services.  

 Still focusing on the elements, there is the aspect of public programs, which ensure that the roles are achieved. This mostly comes with the adjustment of some of the sub programs in order to ensure that there is improved access to proper health care. Additionally, new patient models need to be developed; this is based on the increased number of patients. Since the number of people is large, then it is important to ensure that the models available cater for the patients. From this, the medical terms are addressed, and therefore some of the limitations are avoided.  

 As mentioned earlier, the healthcare affordable act ensures that services provided are more than efficient, and therefore this requires most efficient people to be present. This cuts across the health care providers, in this case, we shall focus on the nurses. To begin with, it is their role to ensure that tests are done in the right procedures, and hence better health care is fostered. This not only offers the faster recovery to the patients but also results in the success of the health facility at large.    Another role comes hand in hand with the treatment services, which are rendered in the facility. The nurses are influential and hence when it comes to the implementation law, they play the role of patient safety. The main aim of the health care models and programs is to improve the traditional system to ensure that patients receive the most affordable quality services.  

 Another role is to ensure that there is coordination; this comes from the aspect of engaging with the other health care providers as well as ensuring that the patients are also involved. This means that without coordination nothing can be done and more so the health system could be subjected to failure. However, with the nurses, the transition is possible, and more so the services offered are not only relevant but also they offer assurance to the patients. More so, when it comes to the roles, the nurses bring out the aspect of responsibility and hence through the engaging done they are in a position to enhance effectiveness.  

 Nurses play the role of a leader, and it is due this that they are able to improve some of the areas, which are not considered essential. Out of this, they not only discharge patients that are better in terms of the health status but also they are able to reform the society. To sum up, the key role of the nurses is to offer efficiency and engage in all efforts that are possible this is important since it ensures that the patients receive their rights as citizens.
